Advancing Digital Coaching for Sustainable Lifestyle Change in Type 2 Diabetes

Risky lifestyle behaviors remain one of the most pressing challenges in global health, contributing to the rising burden of chronic diseases. Despite wide recognition of the need for healthier long-term habits, achieving meaningful and lasting behavioral change continues to be difficult. Traditional interventions often fall short in promoting sustained motivation, and cost-effective digital solutions that adapt to individual needs are still limited.

At Healthentia, we are committed to bridging this gap by leveraging science-driven innovation in digital health. Our latest research presents a holistic framework designed to transform lifestyle behavior change through personalized digital coaching, powered by an automated multiagent reasoning system.

 

A Science-Backed Multiagent Framework

This framework brings together established behavioral change science and the practical strength of Healthentia’s certified SaMD (Software as a medical device). At its core, the system uses multiagent reasoning to select the most effective coaching strategies for everyone based on personal assessments and daily health and lifestyle data.

By prioritizing evidence-based BCTs (Behavioral Change Techniques) with the strongest impact on change, Healthentia’s digital coaching adapts in real time to user progress and engagement patterns. This ensures that interventions are not only relevant but also sustainable, empowering patients to take ownership of their health journey.

 

Application in Type 2 Diabetes Management

To illustrate this approach, we applied the behavioral change coaching scheme in a study involving patients with Type 2 Diabetes during a total of 12 weeks intervention plan with a 2-week assessment and adherence control weeks. Nine participants were monitored using Healthentia SaMD for remote care and personalized coaching content delivery.

The outcomes were highly encouraging. Within the group, fasting glucose levels dropped by an average of –17.3 mg/dL, supported by a large effect size and strong statistical significance. Patients also experienced meaningful reductions in weight (–2.89 kg) and BMI (–1.05 kg/m²). Importantly, the personalized coaching delivered through Healthentia resonated with patients, with over 71% of the opened, recommended content received positively.

These results demonstrate the potential of combining behavioral science with adaptive digital coaching to create measurable improvements in health outcomes.

 

What This Means for the Future

While these findings are preliminary, they highlight a promising path forward. Future directions include refining the multiagent selection process, expanding the type 2 diabetes program with more comprehensive health markers, and broadening the applications of this framework to other chronic conditions.

Further to the CE-marked virtual coaching module of Healthentia, wellbeing coaching support that is supported by genAI technologies can have strong influence to the patients’ behavior. This is to be studied and presented in future work.

Multidomain Behavioral Change Digital Coaching for Chronic Disease Management in Patients With Type 2 Diabetes: Framework Development and Preliminary Evaluation

Background

Unhealthy lifestyle behaviors have been identified as a major cause of numerous health issues, with a steady global increase in their prevalence. Addressing this challenge requires comprehensive behavioral changes to promote the adoption of a sustainable healthier lifestyle. However, despite the prevalent need, cost-effective and successful digital coaching for health-related behavior change remains scarce.

 

Objective

This study aimed to present a holistic framework for designing, modeling, and executing behavior change strategies through a multiagent reasoning system that selected optimal digital coaching techniques based on individual assessments and integrated data-driven decision-making.

 
Methods

Behavioral change theories have been explored to design a multiagent system aimed at achieving sustainable lifestyle changes. This system selected behavior change techniques based on individual user assessments, prioritizing those with the strongest impact on key behavioral components. The framework incorporated evidence-based practices stemming from behavioral change science and integrated them into Healthentia’s behavioral change coaching scheme. Healthentia, a certified software as a medical device, implemented this framework in its non-medical modules that aim for lifestyle behavioral change and wellbeing specifically for chronic disease management, serving as an eHealth solution that advances decentralized care by enabling remote monitoring, data-driven content selection, and personalized digital coaching that adjusts to patient progress and engagement patterns.

 
Results

This study explored the application of the Healthentia behavioral change coaching scheme in patients with type 2 diabetes. Behavioral attributes have been evaluated in 9 patients, yielding notable results in terms of fasting glucose dropping by an average of –17.3 mg/dL (Cohen d=1.5; P=.002), further underscored by a narrow 95% CI (–26.1 to –8.43), and in terms of weight and BMI, with mean reductions of –2.89 kg and –1.05 kg/m², respectively. These changes yielded large effect sizes (Cohen d approximately 1.05) and were statistically significant (P=.01). The positive outcomes were at least partly attributed to the personalized delivery of content, 71.66% (1125/1570) of which was well received by the patients.

 

Conclusions
Our study of this multiagent system, which was tested through simulated patient behavior and preliminary, limited behavior observations of patients with type 2 diabetes, promises improved health outcomes using personalized digital coaching strategies. Future directions include optimizing the multiagent selection process; further exploring the type 2 diabetes program; conducting an in-depth evaluation of its results, including glycated hemoglobin measurements; and expanding its applications to other chronic conditions.

A paper recently was published on the study design and Phase 1 results of the AZIMUTH study representing a successful partnership between leading Italian medical centers, AstraZeneca, and Healthentia, demonstrating how collaborative innovation can transform healthcare delivery. AZIMUTH study leveraged Healthentia’s certified digital health platform to digitally transform heart failure care with remote monitoring, patient engagement, and collaborative innovation. This multicenter initiative leveraged Healthentia Medical Device (SaMD), to address the persistent challenges in heart failure management.

The paper highlighted how the platform addressed the fragmented nature of heart failure care by enabling continuous communication between hospital specialists and community healthcare providers.

Study Design and Implementation

The AZIMUTH study (FondAZione A. Gemelli IRCCS Artificial Intelligence Empowered Digital PlatforM to sUpport paTients with Heart Failure) addressed this healthcare challenge through innovative digital health technology. It demonstrates how smartphone-based care could transform outcomes for heart failure patients. This multicenter, prospective study enrolled 300 heart failure patients across four leading Italian medical centers in Phases 1 and 2.

The study utilized Healthentia v3, a Class I Software as Medical Device (SaMD), as the core platform for remote patient monitoring and care delivery operating through two integrated components:

Patient Mobile Application: Patients used intuitive “widgets” for daily health monitoring, including mandatory weight and blood pressure tracking, validated questionnaires (Kansas Questionnaire and medication adherence assessments), and optional monitoring of heart rate, oxygen saturation, and physical activity. The app seamlessly integrated with Bluetooth-enabled devices to minimize manual entry errors.

Clinical Dashboard: Healthcare providers accessed a platform that consolidated patient data into actionable insights, featuring real-time monitoring, intelligent alert systems based on clinical thresholds, longitudinal trend analysis, and integrated patient communication tools.

The study carefully addressed the real-world implementation by establishing clear inclusion criteria that balanced technological requirements with practical applicability. Patients required basic digital literacy or caregiver support, smartphone compatibility, and received comprehensive training during enrollment. The platform was designed for intuitive use across age groups, ensuring broad accessibility.

Proven Results

The study successfully demonstrated the effectiveness of digital health solutions in heart failure management:

Patient Engagement: Achieved the primary objective with over 70% of patients successfully engaging with the digital platform throughout the study period.

Clinical Improvements: Patients showed significantly improved medication adherence and treatment engagement compared to traditional care approaches.

Healthcare Provider Value: Clinicians reported enhanced ability to monitor patients remotely, identify early warning signs, and coordinate care more effectively.

Feasibility Confirmed: The app-based model proved both technically feasible and clinically valuable across diverse patient demographics.

Impact and Validation

AZIMUTH validated that smartphone-based care could transform outcomes for heart failure patients – a population traditionally challenged by frequent hospitalizations and complex medication regimens. The study proved that digital health solutions, when properly implemented through certified medical device platforms like Healthentia, could enhance both patient outcomes and healthcare efficiency.

The successful completion of AZIMUTH Phase 1 established a new evidence base for digital health adoption in chronic disease management. Azimuth demonstrated that collaborative innovation between healthcare institutions, pharmaceutical companies, and technology providers can deliver measurable clinical value. A second paper with results from Phase II is expected soon and new partners have joined efforts in a new study named Azimusa utilizing the Azimuth care model and expanding further the scope to more medical centers in the north with more patients.

Background

Despite advancements in disease-modifying therapies, the rate of hospitalizations in patients with heart failure (HF) remains high, with an increased risk of future adverse events and healthcare costs. In this context, the AZIMUTH study aims to evaluate the large-scale applicability of a smartphone app-based model of care to improve the quality of care and clinical outcomes of HF patients.

 

Methods

The AZIMUTH trial is a multicentre, prospective, pragmatic, interventional, single-cohort study enrolling HF patients. Three hundred patients will be recruited from four different sites. For comparative analyses, both historical data from participating hospitals for the 6 months before enrollment, along with propensity-matching score analyses from GENERATOR HF DataMart, will be used. The estimated duration of the study is 6 months. During the whole observational period, the patients are asked to provide information regarding their clinical status, transmit remote clinical parameters, and periodically answer validated questionnaires, Kansas City Cardiomyopathy Questionnaire Health and Morisky Medication Adherence Scale 8-item, on a mobile application, through which healthcare providers implement therapeutic adjustments and remote clinical assessments. The primary objective of this study is to evaluate the feasibility, usability and perceived benefits for key stakeholders (patients and clinical staff) of the AZIMUTH digital platform in the enrolled patients when compared to standard of care. Secondary endpoints will be the description of the rate of hospital readmissions, ambulatory visits and prescribed therapy in the 6 months following enrolment in the experimental group compared to both the historical and propensity-matched cohorts.

 

Perspective

The AZIMUTH aims to enhance HF management by leveraging digital technologies to support the care process and enhance monitoring, engagement, and personalized treatment for HF patients.

Software as a Medical Devices (SaMD) and mobile health (mHealth) applications have revolutionized the healthcare landscape in the areas of Remote Patient Monitoring (RPM) and Digital Therapeutics (DTx). These technological advancements offer a range of benefits, from improved patient engagement and real-time monitoring to personalized treatment plans and enhanced clinical outcomes. During the continuous design, development, and validation of our medical device Healthentia, we have conducted a comprehensive systematic literature review to assess the current status and efficacy of these digital health interventions and benchmark the results with our finding from the clinical evidence we have collected from various studies. The results are rather positive, demonstrating how SaMDs like Healthentia can support the management of chronic diseases, satisfying patient safety and performance requirements.

The current landscape of SaMD and mHealth apps is characterized by rapid innovation and widespread adoption, especially after the Covid pandemic, which might have worked as a catalyst for digital transformation in digital health. SaMD refers to software intended to be used for medical purposes that perform these purposes without being part of a hardware medical device. mHealth applications, on the other hand, encompass a wide range of digital tools designed to support health and wellness through mobile devices. mHealth applications are not necessarily medical devices and therefore, they often don’t comply with the regulatory framework. Both SaMD and mHealth apps have seen significant advancements, driven by the increasing need for accessible, efficient, and patient-centric healthcare solutions. Regulatory frameworks have also evolved to keep pace with these innovations, ensuring that these technologies meet rigorous safety and efficacy standards. While the potential of SaMD and mHealth apps are immense, there are several challenges that need to be addressed to fully realize their benefits, such as data privacy concerns, integration with existing healthcare systems, and user adoption barriers.

The review involved a scientific process of identifying, selecting, and analyzing relevant studies that evaluate the effectiveness of SaMD and mHealth apps in managing chronic diseases, using the PICO (Population, Intervention, Comparison, Outcomes and Study) framework. The selected studies provide a robust evidence base, highlighting the positive impact of these technologies on patient outcomes.

This post aims to provide a comprehensive overview of the current status of SaMD and mHealth apps, highlight promising results, and discuss the potential these technologies hold for improving health outcomes and provides only an overview of a peer reviewed paper that we expect to publish in the coming months.

The safety and performances of such solutions for chronic disease patients is thoroughly discussed in various meta-analyses that have been retrieved and analyzed and include 14 meta-analyses for diabetes Type II, cardiovascular diseases, COPD and cancer. Those meta-analyses allow to identify the main clinical performances that could be expected for Healthentia to define the acceptance criteria for the risks/benefits acceptability. In addition, clinical studies originating from the selected systematic reviews were analyzed to check which features were put in place in the eHealth solutions under evaluation, allowing a comparison with those of Healthentia, resulting in prospective controlled studies on several chronic diseases:

  • Diabetes Type II: 24 studies, about 1.200 patients in the intervention group
  • COPD: 1 study, 36 patients
  • Cancer: 3 studies, about 150 patients
  • Heart diseases: 7 studies, 560 patients

Summarizing the results from these studies we provide some data both biomarkers and other clinical endpoints that demonstrate the potential:

  • HbA1c level: Decreased in several of the studies.
  • Physical activity & performance: Significant improvements in exercise capacity.
  • Body fat: Significant improvements.
  • Sleep: Significant improvements in sleep time.
  • Self-care/management: Self-reported improved the self-monitoring of patients’ blood glucose levels, diet, exercise, other self-management skills, and knowledge of the disease.
  • Depression & anxiety: Self-reported improved effect on their psychological status.
  • 6MWD: Significant improvement.
  • (HR)QoL: Difference between the DQoL–Social/Vocational Concerns subscale scores was statistically significant.
  • Hospital readmissions: The 30-day hospital readmission rate was much lower, compared with the national readmission rates.
  • Adherence: Significant improved medication adherence.

The list of results above is indicative and is only provided to showcase the potential and is not to be considered as a scientific outcome itself. Scientific results will be presented and analyzed in the Systematic Literature Review paper in the coming months, which will also include evidence and findings from studies that used Healthentia. By sharing our findings and experiences, we hope to contribute to the ongoing dialogue on digital health innovations and their role in transforming chronic disease management. Furthermore, the insights gained from the literature review and our benchmarking analysis may offer valuable guidance for healthcare providers, patients, and policymakers as they navigate the evolving landscape of digital health.

Looking ahead, the future of SaMD and mHealth applications appears promising, with the potential to significantly enhance the quality of care for patients with chronic diseases.

Among the many transformative solutions emerging in Healthcare, Virtual Wards have garnered considerable attention for their potential to revolutionize patient care. By leveraging digital tools and remote monitoring technologies, virtual wards offer a paradigm shift in how hospitals deliver services and how patients experience healthcare. In this blog post, we will highlight some benefits of virtual wards, exploring their impact on hospitals, patients, and the broader healthcare landscape and give details of our implementation of our recent use cases.

The biggest benefit of these wards are to facilitate continuous monitoring of patients’ health post discharge, reducing readmissions by providing post-discharge support and guidance. Access to care improves, especially for those in remote areas. Cost savings accrue through optimized resource utilization and reduced hospital stays. Patients feel empowered to manage their health with remote tools and education, enhancing their overall experience. Flexibility and convenience are paramount, allowing patients to receive care from home. Moreover, virtual wards support population health management by enabling effective monitoring and intervention based on collected data. Overall, they promise improved outcomes, cost-effectiveness, and patient satisfaction.

Healthentia has been used in two scenarios in Oncology. We will explore here the one from the Gynecology Oncology Unit that offers a direct line between surgeons and patients who will be able to report any symptoms or complications during convalescence.
The continuous observation and assistance to patients, monitoring their health condition outside the hospital, make management and interaction with the clinical team increasingly effective during the post-operative period, help research towards increasingly personalized care models. These are in summary the objectives of the Virtual Ward, a new App dedicated to gynecological patients undergoing surgery at the Gynecology Oncology Unit of the A. Gemelli IRCCS University Hospital, directed by Professor Giovanni Scambia.

The patient who decides to enter the Virtual Ward can download the Healthentia App for free on her mobile phone and thus begin to share numerous information that will help the clinical team to have a complete and updated picture of her health status. The Care Managers present the care path during the interview with the patient admitted to the ward and inform the doctor about the state of health and promptly notify him in case of need and urgency. The process begins at the time of discharge from the hospital and lasts approximately 30 days during which the patient receives a series of questionnaires which aim to evaluate the progress of the patient’s health condition with particular attention to signs and post-operative symptoms, state of the surgical wound and monitoring of any devices, such as catheters or drainage.
The patient also has the possibility of sharing numerous optional data such as vital parameters and multimedia content (photos, videos, screenshots of reports external to the facility), as well as being able to interact with healthcare professionals immediately and continuously.

“Virtual Ward is an integral part of our clinical practice – explains Professor Anna Fagotti , Director of the Ovarian Carcinoma Unit of the Agostino Gemelli IRCCS University Polyclinic Foundation. – This App is useful for patients to share, understand and resolve any health problems that arise while they are at home, accompanying them in the days following discharge from hospital. The home convalescence phase – continues Professor Fagotti – is a difficult emotional moment for patients, there are many uncertainties and many questions about the treatment path. Furthermore, patients may have little knowledge of the disease and difficulty recognizing signs and symptoms of clinical importance.”

The App can be an extremely useful tool, in fact it includes a Virtual Assistant who conveys useful information and advice, for example, relating to the lifestyle to follow in the post-operative period, thus covering an important educational role.
“This tool – concludes Professor Fagotti – is also very useful for us doctors. The data sent by the App allows us to immediately detect the onset of symptoms to correlate with possible complications related to the surgery which otherwise could emerge late. Through the App we can interact with the patient immediately and continuously and intervene in a timely and punctual manner.”

The close collaboration between the healthcare team and the Gemelli Generator – Real World Data Facility, coordinated by Dr. Stefano Patarnello , has made it possible to create a care model capable of integrating and supporting traditional post-operative monitoring means.
The experience of the clinical team made it possible to define an innovative assistance path: the questionnaires were defined to allow the patient an effective and pragmatic interaction; all the information collected allows a holistic view of the state of health; finally, the team of Care Managers dedicated to the development and management of the new care path is the focal point for guaranteeing the provision of the service on a daily basis.
The Gemelli Generator – Real World Data project team, coordinated by Dr. Alice Luraschi , worked together with Innovation Sprint company, manufacturer of the Healthentia medical device, to offer an intuitive service and tool of immediate benefit in clinical practice and in the patient’s daily life .

Study Description

COMFORTAGE is a joint effort of medical experts, social scientists and humanists, technical experts, Digital Innovation Hubs (DIHs) and Living Labs (LLs) to establish a pan European framework for Community-based, Integrated and People-Centric prevention, monitoring and progression managing solutions for dementia and frailty.

Through COMFORTAGE, care services providers will be empowered to create and implement personalized, integrated care measures against dementia and frailty, leading to significant improvements in individuals' well-being and quality of life (QoL). The project also focuses on enhancing stakeholders' health and digital literacy to reduce health inequalities and promote healthy aging in modern communities.

COMFORTAGE will be validated through thirteen pilot studies across eight EU member states. It combines medical/clinical innovations, cutting-edge AI technologies, DIHs, and social innovations to address various aspects of health and well-being. The project emphasizes the integration and management of different data sources, including biobanks, cohorts, medical records, and real-world data from sensors and wearables, in a standardized structure called Holistic Health Records (HHRs).

A Virtualized AI-Based Healthcare Platform (VHP) will be developed to centralize access to AI resources for risk analysis, early diagnosis, and personalized decision-making. This platform will facilitate collaboration among stakeholders and promote effective strategies for health promotion, disease prevention, and treatment, while minimizing digital literacy inequalities. The VHP will also serve as a key tool for the exploitation and sustainability of the project's results. COMFORTAGE aims to establish an ecosystem of healthcare stakeholders and viable business models to ensure the project's long-term impact and wider adoption.

Our role

Innovation Sprint is a technology provider within the COMFORTAGE project. Our focus lies on enhancing the functionality of Healthentia, our flagship product, with a specific emphasis on data collection, gamification, and virtual coaching. Our involvement includes various tasks across different work packages. In Work Package 4, we translate pilot requirements into precise secondary data collection and virtual coaching specifications. This involves adapting the Healthentia platform to accommodate the project's secondary data collection needs, allowing for a deeper understanding of patients' habits and connections, and offer content to the end-users regarding their progress towards healthier habits, through our coach companion. Furthermore, in Work Package 5, we concentrate on developing the gamification features of the Healthentia mobile app. This includes the creation of engaging elements to enhance user experience and encourage active participation. Moreover, we collaborate closely with other partners, to co-develop and host serious games designed to improve motor and cognitive skills. These games enhance the overall experience of the users and promote their skill development. Lastly, we actively contribute to the dissemination and exploitation efforts outlined in Work Package 8, ensuring that the outcomes of our work reach relevant stakeholders and are effectively utilized.

Managing insulin therapy for type 2 diabetes can be challenging due to the need for frequent dose adjustments. This study examined whether a voice-based AI assistant could help patients achieve better blood sugar control by adjusting their insulin dosages more effectively.

In an RCT presented by Ashwin Nayak et al in JAMA Netw Open. 2023;6(12): e2340232, titled Use of Voice-Based Conversational Artificial Intelligence for Basal Insulin Prescription Management Among Patients With Type 2 Diabetes, is presented the effectiveness of voice-based conversational AI applications in facilitating swift optimization of basal insulin dosage for individuals with type 2 diabetes.

The study involved 32 adults with type 2 diabetes who either used the AI assistant or received standard care. Those who used the AI assistant reached the optimal insulin dose significantly faster (median of 15 days vs. over 56 days for standard care). Additionally, AI users had better insulin adherence (83% vs. 50% for standard care), improved glycemic control, and reduced diabetes-related emotional distress.

These findings suggest that voice-based AI assistants can be a valuable tool for type 2 diabetes patients, helping them achieve better blood sugar control and improve their overall well-being. By providing personalized and adaptive insulin dosing recommendations, voice-based AI assistants can empower patients to take control of their condition and achieve better health outcomes.

The ability to quickly reach the optimal insulin dose, enhance adherence, and reduce emotional distress highlights the transformative impact of this technology in improving the lives of individuals with type 2 diabetes.

As AI continues to advance, its role in healthcare is expected to expand significantly. Voice-based AI assistants like the one used in this study offer a promising approach to medication titration and personalized treatment guidance, holding the potential to revolutionize chronic disease management and improve patient outcomes.
